The eye exam may be provided at no cost when the person is eligible and the required notation appears in the booklet.
Glasses or lenses
Assistance for the purchase or replacement of glasses or contact lenses may apply, subject to authorization.
Vision770 offer
Vision770 may cover the applicable difference on a specific selection so a complete pair can cost $0 for some eligible individuals. Options outside this selection may incur fees.
Valid health insurance card
Your RAMQ card must be current.
Valid claim booklet
Bring your booklet with the current month's coupon attached.
Services Québec (MESS) form or authorization, when required
For glasses assistance a specific authorization is generally required.
Recent prescription, if you have one
